Work Hours: Full time, 40 hours per week
Role Type: Permanent
Location: Based in Porirua, Wellington
 
Vaka Tautua is a Pacific non-government organisation providing services nationally for Pacific people who experience mental illness or disability. We are looking for a new full time Mental Health Navigator to join our Mental Health Navigation Team based in Porirua. This position will work with our mental health consumers to provide the community support services which promote their
personal recovery.
 
Our services follow the Popao model and are aimed at strengthening independent living through promoting self-improvement, self-help, destigmitisation, social networking and advocacy. The Mental Health Navigator provides coordination between the consumer, their family, clinical teams, and the community by delivering a flexible plan which meets the changing needs of the consumer.
 
This is important and rewarding work. Your experience in working in mental health and your ability to relate to all and contribute as part of a team will be key to your success. 

Role Responsibilities:
 
1. Professionally manage a case load of consumers in a down-to-earth and non-judgemental way to ensure they grow their independence.
2. Diligently manage data files for all your consumers, keeping them up to date and accurate, and ensuring SNAP plans are completed.
3. Network with referring organisations as directed by the Team Leader.
4. Deliver services in a culturally appropriate and competent manner, ensuring the integrity of each consumer's culture is acknowledged and respected.

Desired Attributes:
 
• Experience managing a case load in mental health and the ability to self-manage workload to meet targets.
• The ability to facilitate group activities and network with stakeholder groups.
• Excellent communication skills, both written and verbal, and fluency in a Pacific language.
• A National Certificate in Mental Health and Addiction Support Level 4, or be studying or willing to study towards this qualification.
• Good computer skills in Outlook and Word.
• A full driver's license.
